[LTP] [PATCH 0/2] Add ground rules doc page

Changes in v4:

- fixed process state doc (Petr)

- changed the manpage links to match the new format

- adjusted "Split changes" paragrap a suggested by Li

- fixed heading levels (so that headings show up properly in menu)
